The Your Care Connect platform (the "App") is operated by Ontario Shores Centre for Mental Health Sciences (Ontario Shores). At Ontario Shores, we are committed to respecting and protecting the privacy of your personal information.

Our privacy policy’s ("Privacy Policy") main goal is to describe our data collection, sharing, and protection practices. The terms "You," "Your," and "Yours" refer to the user/customer using the App. This Privacy Policy is overseen by Your Care Connect Terms & Conditions.

1. Users

The App is intended for current Ontario Shores patients.

2. Collection of Information

"Personal Information" means any information which can be used to identify an individual, and may include, but is not limited to, your name and email address. Ontario Shores limits the collection of this information which is needed to achieve the purposes as described in the "Information Use" section below. Any Personal Information received by the App from You will be treated in accordance with this Privacy Policy and Ontario Privacy law.

3. Information Use

Ontario Shores uses Personal Information for several general purposes:

    i. To provide You with service information, and notifications;
    ii. To support the services offered on the App;
    iii. To contact You for feedback about our services;
    iv. To conduct research; and,
    v. to notify You of technical updates or changes in policy.

4. Information Sharing and Disclosure

We will not sell or rent Your Personal Information to other people or companies except as described in this Privacy Policy. We may use Your Personal Information under the following circumstances:

    i. If sharing the information is reasonably necessary to provide a service that You have requested;
    ii. To keep You up to date on the latest software updates;
    iii. If we are required to do so by law;
    iv. If we receive consent to share Your Personal Information;
    v. If any of Your Personal Information is removed so we can gather information about the Service to inform Service changes.

6. Security

Ontario Shores puts safety measures in place to protect Your Personal Information from loss, misuse, disclosure, alteration, unauthorized access, and destruction. Ontario Shores protects the security of Your Personal Information when being sent to us by using Secure Sockets Layer software, which encrypts the information You input in the App.

7. Third Party Apps

The App may contain links to other Apps. Ontario Shores is not responsible for the privacy practices of other Apps. When leaving the App which may be linked to another App, read the privacy statement about the collection of Your Personal Information.

8. Your Consent to this Privacy Policy

By using this App, You agree to this Privacy Policy. If we update this Privacy Policy, the changes will be dated and posted on Ontarioshores.ca.

If we decide to use Personal Information in a manner different from that stated at the time it was collected, we will notify users via email (if an email address has been provided) or display a notice on Ontarioshores.ca.
